The Hancock County Health Department inspects restaurants four times annually and violations are determined as “C” for critical, “NC” for noncritical and “R” for repeated violations. The following inspections are based on information obtained from Retail Food Inspection Reports.

June 3

Zapp Thai Mexican Restaurant

Location: 2045 N. State St., Greenfield

Type of Inspection: Routine

Violations found: C — prep cooler across from woks at 60 degrees, corrected; C/R — need to have gap under three compartment sink, to be corrected by July 1; NC — employee used food prep sink for hand washing, corrected; C/R — food contact surfaces must be sanitized after rinsing, corrected; NC/R — provide test strips for sanitizer, to be corrected by July 1; NC — improper cooling method for rice observed, to be corrected by July 1.

June 4

Bonsai Grill

Location: 18 S. Main St., Fortville

Type of Inspection: Routine

Violations found: C — hand sink being used for purposes other than hand washing, debris on ice scoop where drink boxes are located, dish machine sanitizer concentration is less that 50 ppm, corrected; NC — debris on top of top loading cooler in bar area and on carts by fryer in kitchen, need self-closing doors at restrooms, to be corrected by July 4; order of washing in three-compartment sink in bar area is wash, rinse, sanitize, air dry, corrected.

June 8

Dave’s Meat Market

Location: 333 S. State St., Greenfield

Type of Inspection: Routine

Violations found: C — mayo used for sandwiches at 58 degrees, shell eggs are mislabeled, corrected; C/R — no date on ham salad in walk-in cooler, corrected; NC/R — need to have food thermometer in all refrigeration units, dirt/debris up along edges, under equipment floors, to be corrected by July 1; NC — trash receptacles have food debris, to be corrected by July 1; C — racks where clean dishes are stored need to be cleaned, to be corrected by June 9.

Wendy’s

Location: 1508 N. State St., Greenfield

Type of Inspection: Routine

Violations found: C — cheese on lines are at 50 degrees or above, no time mark on cheese, raw bacon open in walk-in cooler, some bacon touching box, employee observed touching walk-in cooler handled and face mask with gloves on and no changing gloves/washing hands before handling ready-to-eat food, corrected; NC — no paper towels at hand sink up front, corrected.

Kroger & Fuel Center

Location: 1571 N. State St., Greenfield

Type of Inspection: Routine

Violations found: C — rotisserie chicken hot holding at 130 degrees (needs to be held at 135 degrees or above), corrected; C — hand sink for sushi prep needs to be closer than 25 feet with no barriers such as doors, walls, etc., to be corrected by July 11; C/R — handles of deli slide doors need to be treated as food contact surfaces, sanitize every four hours if going to touch sliding doors then touch ready-to-eat food, corrected.
