Location: 426 E. Broadway St., Fortville.
Type of inspection: Routine.
Violations: NC/R — clean floors, wall, shelving where food/debris/grease has accumulated, especially legs of equipment, under equipment, etc. Clean out closet with brooms and mops, 90 days; NC — repair walls and covering so they are smooth, cleanable and nonabsorbent, 90 days; NC — clean outsides of food storage containers such as those holding flour, cabinets holding utensils, shelving, etc., 90 days; NC — replace light rods that are out, 90 days; NC — get rid of stuff no longer used or unnecessary to food operation, 90 days.

Also, condensation dripping from pipe in cooler — do not do any food prep under dripping pipe, 5 days; C — need to wash hands after touching face, clothing, etc. Must have clean hands before putting on gloves — don’t touch clothing, hair, face, etc. with gloves on, corrected; NC/R — continue to clean under and behind equipment, 90 days.
