Violations found: C — must have certified food protection manager on premises, to be corrected by Nov. 1; C — nacho cheese and chili dispenser must hold hot foods at 135 degrees or higher, corrected; C/R2 — serving utensils/tongs must be cleaned and sanitized at least every four hours when in used, corrected; NC — must have sanitizing test strips available and used, to be corrected by Sept. 15.

Location: 5965 W. Broadway, McCordsville
