The Hancock County Health Department inspects restaurants four times annually and violations are determined as “C” for critical, “NC” for noncritical and “R” for repeated violations. The following inspections are based on information obtained from Retail Food Inspection reports.

Dec. 2

Fortville Broadway Diner

426 E. Broadway St., Fortville

Violations found: C – hand washing sink needs to be accessible at all times, corrected; C – employee needs to wash hands before putting on food handling gloves after touching face, corrected; NC – ceiling, walls and floors have debris throughout the kitchen area, to be corrected by Dec. 6; NC – shelving holding to go to containers, shelving below grill and throughout kitchen has debris, to be corrected by Dec. 6; NC – replace cabinet holding utensils by sandwich prep cooler; NC – ceiling in kitchen above steam table is coming down.

Violations found: NC – floors throughout bar and sushi area, waitress areas, kitchen and back door have debris, grease build-up under equipment, in corners and edges, to be corrected by Dec. 13; NC, R – equipment and shelving in sushi/bar area has build-up of debris, to be corrected by Dec. 13; C, R – knives on magnetic strip that are considered clean have debris, corrected; C, R – rice is below hot holding temperature of 135 degrees, corrected; C – dish machine sanitizer concentration is too low, corrected; NC – need to provide food thermometer with small diameter probe to use with thin foods, to be corrected by Dec. 13; NC – wash cycle of dish machine did not reach 120 degrees, to be corrected by Dec. 13; NC – employee observed washing hands at three-compartment sink, corrected; NC – shelf unit in kitchen between dish machine and three-compartment sink is in disrepair, repair or replace by Dec. 13; NC, R – splash guard needs to be installed at mop sink and above stove, some paint and wood surfaces are not smooth and cleanable, to be corrected by Dec. 13.

Violations found: NC, R – floors under oven/fryer have debris, debris on wall by pizza oven, to be corrected by Dec. 14; NC, R – tiles cracked and broken in places throughout kitchen, to be corrected by Dec. 31; NC – debris on shelving above food prep coolers and on dish drying rack, to be corrected by Dec. 14.

Violations found: C – spray bottle of liquid does not have identification labeling, corrected; C – cheese sauce at 95 degrees, needs to be held at 135 degrees or higher, discarded; C – need air gap at three-compartment sink, to be corrected by Jan. 7.

Violations found: NC – need to provide paper towels at bar area hand sink, corrected; NC – need to provide thermometers in all refrigeration units that do not have external temperature reading, to be corrected by Jan. 27; C – chicken wings with bone did not reach internal temperature of 165 degrees, corrected; C – dish machine is not dispensing sanitizer, corrected; C – employee observed cutting lemons without gloves, corrected.

Violations found: C, R – homemade ranch dressing and pre-cooked open chicken do not have date mark, corrected; NC – debris build-up in walk-in cooler on floor under storage rack, to be corrected by Feb. 15; NC, R – debris build-up on outside of dough trays and top of lids for dough trays, to be corrected by Feb. 15; NC – need to provide paper towels at hand sink, corrected; NC – lid not on pickles in walk-in cooler, corrected.
