FORTVILLE – Starting Feb. 6 and ending April 12, tax assistance sponsored by AARP will be available at the Fortville Public Library for 2017 federal and state income tax returns. Appointments are available for Tuesday afternoons, Wednesday afternoons and evenings and Thursday afternoons. To make an appointment, call 317-485-6402 or stop by the library, 625 E. Broadway St., Fortville.
Clients must bring:
- Primary and secondary taxpayer photo ID
- Social Security or ITIN cards for taxpayers and all dependents claimed
- Last year’s state and federal tax returns
- W-2 and 1099 forms
- Documents showing interest and dividends received
- Investment funds documents
- Social Security Administration Form 1099-SSA
- Any other documents showing income received and itemized deductions
- Documentation for any state tax refund received during the year
- Information regarding federal and or State quarterly tax payments
- Renters please bring landlord’s name, address and rent amount
- Form 1095-A, B, or C for the Affordable Health Care Statements
- Proof of bank account routing and account numbers for direct deposits
